Immunoglobulin A nephropathy (IgAN) is a common primary glomerulonephritis presenting frequently as recurrent haematuria and infrequently as nephrotic syndrome (NS). Horseshoe kidney (HSK) is the most common renal fusion anomaly with many renal and extra renal structural abnormalities. Although, IgAN and HSK occur frequently on their own, their combination is uncommon. There are only a few reported cases of glomerular diseases in HSK, of which IgAN is rare. We report a rare case of IgAN presenting as NS in a 10-year old boy with HSK.
