Oro-antral communications are the common sequelae of dental extractions involving maxillary teeth. Smaller oro-antral communications heal spontaneously; however larger communications undergoes epithelization to form oro antral fistula. A female patient reported to the department of oral and maxillofacial surgery, and complained of leakage of oral fluids from her nose. In this case, for closing the oro antral fistula, the conventional press fit technique was modified by using L-shaped locking monocortical titanium bone plate to fix and stabilize the bone graft along buccal fat pad and oral mucoperiosteal flap. Modified press fit technique is recommended by the author for achieving stability of bone graft when it’s not achieving a snug fit.
