Prosthodontic management of severely worn dentition using pankey-mann-schuyler philosophy with functionally generated pathway- A case report

Abstract

Attrition is defined as a loss of tooth structure caused by tooth-to-tooth friction without any intervening substance. Occlusal and incisal attrition may occur during deglutition (physiological wear) and may be severe if associated with parafunctional activities such as bruxism and clenching habits (pathologic wear). Shiny dental surface and well-defined facets are considered as reliable signs of attrition that usually present with facets on antagonizing teeth. Severe tooth wear is frequently multifactorial and variable. Successful management is a subject of interest in dentistry. Management of patients with worn dentition is complex and difficult. Accurate clinical and radiographic examinations, a diagnostic wax-up, and determining OVD are crucial. Apart from esthetic challenges, the dentition needs to be carefully evaluated for biological and mechanical reasons. Several techniques were formulated as a management for attrited dentition to meet esthetics and functional harmony. Full-mouth rehabilitation is a dynamic functional endeavor, and it embodies the correlation and integration of all component parts into one functioning unit. The management of tooth wear, especially attrition has become a subject of interest in the Prosthodontic literature, both from a preventive and a restorative point of view. The rationales for altering vertical dimension include aesthetic, correcting occlusal relationship, and allowing space for restorations for prosthetic convenience. This case report describes the management of worn dentition in a self reported bruxer who presented with occlusal wear and concomitant non-carious cervical wear. Pankey-Mann-Schuyler philosophy along with functionally generated pathway technique was employed to achieve optimal results.
