Implant placement is a challenge in atrophic posterior mandibular region. Inferior alveolar nerve lateralization (IANL) is a preferred procedure in such cases as it allows longer implant placement. Neurosensory deficit is the most common postoperative complication of this procedure. There are various strategies, which reduce the potential nerve damage involved in IANL. The use of piezoelectric device over rotary device minimizes accidental injury to neurovascular bundle. The repositioning of nerve after implant placement involves a technique of barrier placement between the IANL and the implant in the form of collagen membrane or bone graft. This case report aims to describe IANL technique using piezoelectric device with simultaneous dental implant placement and bone graft as barrier for posterior rehabilitation of an atrophic mandible. This article also compares the three different techniques of nerve repositioning and barrier placement.
