Mandibular second premolar usually exhibit the basic one root and one canal anatomy. This case report describes successful endodontic management of bilateral mandibular second premolar with two separate roots with an uncommon mesial and distal morphological location, which leads us to conclude that clinician should be aware of variations while treating mandibular premolars.
