While replacing missing teeth, our primary goal should be to restore function and preservation of oral tissues and aesthetics. Prosthodontic rehabilitation of distal extension partially edentulous situations remains challenging. The support of distal extension RPD is derived from edentulous ridge and terminal abutment teeth. This case describes the altered cast technique of fabricating removable partial dentures. The advantages of the altered cast technique includes improved stress distribution, decreased food impaction, decreased torquing of the abutment teeth and preservation of the oral structures, all of which leads to patient satisfaction. This technique allows the ridge, recorded in a functional form, to be related to the teeth so that when the prosthesis is seated, it derives support simultaneously from the teeth and the denture base.
