The success of root canal treatment depends on the precise detection of all the canals thorough an appropriately designed access cavity. Failure of the following steps in root canal treatment such as chemomechanical preparation and obturation may occur due to uncommon tooth morphology. Therefore, understanding the various unusual anatomical representations is of paramount importance. Radix Entomolaris the presence of a distolingual supernumerary root found in the first, second, and third mandibular molars. This case series describes successful endodontic management of a series of four cases of this rare anatomic variation.
