With the advent of adhesive restorations modern dentistry has been transformed into a minimally invasive and conservative modality no longer dependent on excess tooth structure removal for retention. Though it has many strengths adhesive dentistry presents with a new set of problems, the biggest of which is instability and degeneration of the adhesive dentin bond. This review article focuses on the reasons for the failure of resin-dentin adhesive bonds like release of matrix metalloproteinases (MMPs) and cysteine cathepsins and discusses various protocols (ethanol wet bonding) and materials (proanthocyanidins, cross-linkers, MMP-Inhibitors etc.) which can be used to increase the longevity of these bonds and ensure long-term stable restorations.
