Evaluation of prognosis of dental implants in diabetic and hypertensive patients

Abstract

The aim of this study is to evaluate the impact of diabetes and hypertension on the prognosis, success, and complications of dental implants, in order to develop effective management strategies for these patients.

A total of 40 patients, aged 35 to 65 years, were enrolled, comprising 15 men and 25 women. 50 implants were placed during the study. Comprehensive patient histories were taken, and laboratory investigations were performed, including HbA1c level assessments. The results indicated a correlation between HbA1c levels and the ratio of complications to the number of implants. Hypertensive patients who were without antihypertensive medicationsand on antihypertensive medication were also included in the study to assess the impact of these medications on implant outcomes. Clinical data, such as the type of antihypertensive drugs used and blood pressure levels, were recorded and analyzed to understand their potential effects on the success, complications, and long-term stability of dental implants. Data analysis was done using SSPS software.

The study included 40 patients with 50 implants, with a total of 6 failures, resulting in an overall implant failure rate of 12%.Additionally, the study examined the impact of hypertension on implant survival in 30 patients. Among non-hypertensive patients, 5 implants (62.5%) survived, while 3 implants (37.5%) failed. In hypertensive patients on antihypertensive medications, 4 implants (80%) survived and 1 implant (20%) failed. For hypertensive patients not on antihypertensive medications, 6 implants (85.7%) survived and 1 implant (14.3%) failed. The p-values for both hypertensive groups, with and without antihypertensive medications, were 0.01*, indicating a statistically significant difference in implant survival rates.

The presence of hypertension and diabetes, as well as the administration of antihypertensive medications, seems to affect the survival rates of implants. Notably, favourable results have been reported in patients who are on antihypertensive therapy and have well-managed diabetes.
