Successful root canal therapy (RCT) hinges on thorough biomechanical and chemical preparation of the root canal system. Accurate working length (WL) determination is fundamental to this process, as it ensures effective cleaning and obturation without damaging periapical tissues.
To evaluate the importance and challenges of precise working length determination in endodontic treatment and its impact on treatment outcomes.
This study reviews and synthesizes findings from multiple clinical investigations examining the accuracy of radiographic versus other techniques (such as electronic apex locators) in determining the WL. The clinical relevance of reaching the apical foramen during both instrumentation and obturation was critically analyzed.
The reviewed literature consistently shows that endodontic success rates improve significantly when the working length extends up to the apical foramen. Radiographic methods, although widely used, were often found to be imprecise due to anatomical variations and image interpretation errors. Alternative techniques, such as electronic apex locators, have demonstrated higher reliability in determining accurate WL.
Accurate determination of working length is a critical determinant in the clinical success of root canal therapy. While radiographic techniques remain standard, their limitations necessitate the integration of more advanced and precise tools to enhance endodontic outcomes.
