Osteomas of the oral cavity are rare benign osteogenic tumors that typically present as asymptomatic, slow-growing masses. While bleeding is an uncommon presenting symptom, it may occur due to mechanical irritation or surface ulceration of the overlying mucosa.A 30-year-old male presented with a chief complaint of intermittent bleeding from the gingiva in the posterior mandibular region of 6 months duration. Clinical examination revealed a firm, non-mobile, lobulated mass measuring approximately 2.5 cm in diameter w.r.t mandibular left third molar region. Radiographic examination showed a well-defined, radiopaque mass with a characteristic “ground glass” appearance. Histopathological examination confirmed the diagnosis of osteoma with fibroadipose tissue underlying bony trabaculae and intertrabecular spaces showing loose fibrous stroma. Complete surgical excision was performed under local anesthesia with primary closure. The patient showed complete healing at 2-week follow-up with no evidence of recurrence at 6-month follow-up. Complete surgical excision remains the treatment of choice with excellent prognosis and minimal risk of recurrence.
