The zygomatic bone acts as a key lateral midfacial buttress, and fractures of the zygomatic arch may result in significant functional and aesthetic compromise if inadequately treated. In this case, a 38-year-old male with a displaced zygomatic arch fracture was successfully managed by open reduction and internal fixation using a 1.5-mm titanium L-shaped miniplate through Keen’s intraoral vestibular approach, achieving anatomical reduction with restoration of facial symmetry and mouth opening. This report highlights that ORIF via an intraoral approach is an effective, cosmetically acceptable, and reliable technique for managing displaced or delayed zygomatic arch fractures when guided by appropriate imaging and individualized treatment planning.
