Inflamed cysts of maxillary antrum are distinct but clinically significant maxillary sinus pathology.Because the maxilla is predominantly composed of cancellous bone, pathological lesions can more easily expand within the osseous space and extend into adjacent anatomical structures, particularly the maxillary sinus, often occupying and reducing it to a narrowed cavity. Infection of the cyst is the most common complication, and chronic intraosseous suppuration may lead to spontaneous drainage through the formation of a fistulous tract. We hereby report a case of inflamed cyst of maxillary antrum leading to Oro-antral fistula formation, which was successfully treated by cystic enucleation and fistula closure was achieved by using buccal fat pad and palatal connective tissue graft. Healing occurred uneventfully and the patient is on regular follow-up since 3 months.
