Nonsurgical retreatment and periradicular surgery are not always viable solutions to endodontic disease. This case report describes the endodontic treatment and intentional replantation of a maxillary right central incisor with follow-up. Clinical and radiographic analysis suggested perforations at cervical and apical thirds. To manage this intentional replantation of tooth was utilized. Incidentally tooth had a severe dilaceration which would have compromised outcome in alternate treatment plans.
