Multidisciplinary approach in the treatment of pathologic migration of lower anterior teeth: A case report

Abstract

Orthodontic treatment in periodontally compromised patients with pathologic migration requires multidisciplinary treatment approach to achieve successful rehabilitation of esthetics and function. Evaluation of etiologic and the contributing factors are important when deciding on appropriate treatment procedures for patients with pathologic migration. If the pathologic migration and bone loss is due to the malposition of teeth, orthodontic treatment needs to be considered with or without periodontal surgery. If periodontal factors are main cause then periodontal plastic surgical procedures are must to achieve successful results.

Following presentation encloses a case report which describes the multidisciplinary treatment in 21 year old female patient with class II div 1 malocclusion with increased over jet and large lower midline diastema associated with pathologic migration and gingival recession of lower anterior teeth. Fixed orthodontic treatment with extraction of upper first molar and periodontal plastic procedure (PPS) such as vestibuloplasty and frenectomy was done to restore facial and dental esthetics.

With increasing demand to improve their general quality of life, patients with compromised periodontal support also seeks orthodontic treatment to improve esthetics and function. Dental treatment for these patients are complex and challenging. However multidisciplinary team approach with motivation of patient for maintenance of oral heath by routine oral prophylaxis is the key for successful rehabilitation
