Odontogenickeratocysts (OKCs) now called as keratocysticodontogenic tumors are the odontogenic cysts of epithelial origin. The collateral variant of keratocystic odontogenic tumor is usually misdiagnosed for inflammatory lesions of endodontic origin or for lateral periodontal cysts.i.e. adjacent to the roots of mandibular premolar/molar teeth. This case report aims to present and elaborate the detailed clinical, histopathological and radiographic features of this variant, which is a rare occurrence.
