Multiple myeloma is a systemic B-cell lymphoproliferative disease that causes osteolytic lesions in the vertebra, ribs, pelvic bone, skull and jaw 10. Multiple myeloma manifestations in the head and neck are common, but usually occur on the late stages of the disease; mandibular involvement as the initial presenting sign of the disease is extremely rare11. This is a case report of 65-year-old female complaining of a swelling on the right mandibular body area since 1 year. A panoramic radiograph revealed an well-defined radiolucent bony lesion involving the right mandibular body. The provisional diagnosis was made out to be as residual cyst, but after careful biopsy the histological examination showed the mandibular lesion to be a plasmacytoma, and a systemic work-up was obtained to rule out multiple myeloma.
