With the development of miniature sensors for measuring body parameters, either by getting attached to the body or part of clothing items, enormous options of monitoring a person's health and activity has been opened. This advancement in the field of medical science is indispensable for design smart home based Person's Activity Monitoring System (PAMS). In this paper, we are designing a multimodal platform which not only measures the vital health parameters of the patient/person via wearable sensors but also monitors the daily activity to provide pro-active health care system at home using fuzzy logic based decision system. To identify person's activity and critical health condition, it gathers body parameter values via wearable medical sensors and recognizes the daily activity of the person with the help of microphones and Infra-red sensor. Thus, it helps in finding the daily activity of the person along with vital health parameters for health care system.
