This procedure was prospectively evaluated in 30 paitents (ASA I toIV) undergoing elective and or emergent orthopaedic surgeries. Those paitents who could not position themselves for sciatic block(i.e. with fracture femur head, hip)were excluded from the study. All 30 cases were given sciatic nerve block through classic posterior approach and “3 in 1” block through inguinal perivascular approach with success rate of 90%. This technique could avoid centroneuraxis block in haemodynamically unstable paitent. Thus it is safe,simple and effective alternative to spinal anesthesia for lower limb surgeries especially in paitents with polytrauma. However one should be careful while using it in paitents having gross hepatic or renal disorder for fear of accumulation upto toxic doses of local anaesthetics.
