Knowledge of dental anatomy is an important tool for successful endodontic practice. A clinician is required to have an insight of morphology of tooth related to its shape, form and structure before commencing treatment. Mandibular molars can have an additional root located lingually (the radix entomolaris) or buccally (the radix paramolaris). This case report discusses endodontic treatment with bilateral radix entomolaris.
