The Outcome of Dental Implants Placed in Narrow Edentulous Ridges with Osteotome Technique: Results of a Clinical Study

Abstract

To identify changes in the buccal-lingual width of the alveolar ridge after ridge expansion with implant insertion.

This study was conducted in patients with missing teeth in the anterior region of maxilla where Implant therapy with bone expansion procedure was the elective treatment to restore partial or total edentulism but the alveolar ridge width was inferior to the optimally planned implant diameter (Ø 3.75–5mm). 10 patients were chosen for the study which required placement of implants in the anterior maxillary region in which ridge width was deficient but not less than 3 mm. Ridge expansion was done using osteotomes. Changes in the buccal-lingual width of the alveolar ridge before osteotomy preparation, at the time of implant insertion, and implant uncovery surgery using a surgical caliper were evaluated.

There was significantly increased in ridge width after implant placement with osteotome technique in all the subjects at all-time intervals. Average pre-operative buccolingual width was 3.15 ± 0.55 mm, mean total gain in width was 1.90 mm. There was loss of one implant but that too due to excessive torque applied during abutment tightening.

The technique of bone expansion with osteotome offers the possibility of placing implants in cases of inadequate bone volume buccolingually for placement of desired implant without the need for other more complex treatments. In addition, it improves the quality of bone surrounding the implants and reduces the time to rehabilitation. This study furnished promising results with regard to placement of implants combined with the osteotome technique in the maxilla.
