The aim of the present study was to compare the influence of two flap designs on periodontal healing and complications after extraction of impacted third molars.
50 patients were randomly selected for the study to compare between comma incision and standard incision in impacted third molar surgery, to evaluate its influence on post operative complication. The individuals were randomly divided into 2 groups.
Comma Incision Group
Standard Incision Group.
Time taken from incision to exposure was less in cases of comma incision as compared to standard incision. The pain scores on operative day, Day 1, Day 3 and Day 7 in the surgical area with comma incision were significantly lower compared to the pain in the area where standard incision was given. Swelling was same with comma incision as that with the standard incision. There was significant difference in mouth opening between the two incisions post operatively. Mouth opening was better in the group 1 in which comma incision was given. Periodontal status respectively though a significant difference was found in probing depth on the 56th post operative day. No significant difference was achieved on all the post operative days in relation to lingual nerve damage, infection, presence of dry socket, wound dehiscence.
From this study we conclude that the new incision design is preferable to the conventional method considering the less degree of post operative complications encountered. Further research with newer flap designs, which will minimize the post operative complications, should be considered in the extraction of impacted third molar surgery.
