Anomalous origin of left coronary artery from pulmonary artery is a rare congenital coronary anomaly which usually becomes symptomatic in infancy. However, in few cases due to extensive collaterals from right coronary artery, they remain asymptomatic till adult life. Later they may present in cardiac failure, arrhythmias or sudden cardiac death. We present here a 52 year old female who came in cardiac failure and on evaluation was found to have this coronary anomaly. She was referred for surgical correction.
