Tranexamic acid (TxA), a synthetic derivative of the amino acid lysine, possesses antifibrinolytic activity. It binds to plasminogen, blocks it's binding to fibrin, and subsequent activation to plasmin This, has been approved for many years for the treatment or prophylaxis of bleeding episodes in patients with bleeding disorders or following major operative procedures via intravenous or oral routes. Its effectiveness in controlling local bleeding from mucosal tissue in the nose, colon, rectum and mouth led us to evaluate its activity in pulmonary bleeding.
Present study aims to know effectiveness of bronchoscopic instillation of tranexamic acid to control the resultant biopsy-induced bleeding.
Study was done in 70 patients. The study was carried out as a prospective randomized trial after taking informed, written consent from patient. Randomization was carried out with random-number tables. A total of 35 patients were selected with aim to treated with the endobronchoscopic instillation of transexamic acid to control biopsy induced bleeding. In control,cold saline was used.
The study was carried out on 70 patients;the patients were divided into two groups of 35 each. In group 1(Tranexamic acid) 35(100%) patients got immediate control of bleeding and bleeding remained controlled after 24 hours also. In group 2 (cold saline) 34 patients had bleeding control immediately and bleeding remained controlled after 24 hours also. Except in 1 case where bleeding was not controlled immediately with cold saline.
Tranexamic acid revealed no clinical significant difference at 1% level of significance. Bleeding controlled in all subjects. No adverse effect seen with tropical use of tranexamic acid.
No significant adverse effects have been noted during the local use of tranexamic acid. Thus in our study we found that tranexamic acid is an effective method for controlling bleeding in patients with significant endobronchial bleeding with lower volume as compared to normal saline and causes no bradicardia.
