Second branchial arch anomalies such as cysts, sinuses and fistulae are the most common congenital defects among other branchial arch anamolies. Among them, complete branchial fistulas with external and internal opening are rare. We present a case report of complete branchial fistula confirmed by fistulogram and excised by combined approach of the fistula by both transcervically and transorally. The pathogenesis, anatomy, clinical features, diagnosis, investigations, histopathology and treatment are discussed in detail.
