Changing Trends in Antibiotic Resistance Pattern of Uropathogens with Reference to Nitrofurantoin, Quinolones, Aminoglycosides and Carbapenems in a Tertiary Care Hospital

Abstract

Urinary tract infection remains one of the most common bacterial infections encountered in clinical practice. Antimicrobial drug resistance is one of the major threats due to widespread use of antimicrobial drugs in general population. The present study was undertaken to report the current antibiotic resistance Pattern among common bacterial uropathogens isolated in a tertiary hospital in Bangalore with reference to nitrofurantoin, quinolones, aminoglycosides and carbapenems.

A cross sectional study was done over a period of 6 months from July 2014- Dec2014. This was an analysis of data generated from the records of consecutive urine samples received in the laboratory during the study period. All the urine samples both from inpatients and outpatients were processed by standard methods and cultured by using urochrome agar. Antibiotic susceptibility Pattern was done by Kirby-Bauer disc diffusion method.

Out of 1642 consecutive urine samples, culture positive cases of urinary tract infection were included for the analysis. About 10 different species were identified. E.coli was the most common uropathogen followed by Klebsiella, Enterococcus, and Proteus, Coagulase negative Staphylococcus, Pseudomonas, Acinetobacter, Citrobacter and Enterobacter. Comparative study was done for antibiotic susceptibility among aminoglycosides, nitrofurantoin, carbapenems and quinolones. The highest resistance was found among aminoglycosides gentamycin (45.3%), and amikacin (33.8%) followed by quinolones like ciprofloxacin (45.8%), norfloxacin (78.5%). Carbapenems (20.25%) and nitrofurantoin (29.6%) did not show much of resistance.

The present study identifies the changing etiologic trends of UTI and a steady increase in development of antimicrobial resistance especially against drugs which were effective. Nitrofurantoin and Carbapenems were found to be most susceptible drugs against isolated uropathogens.
