Metallobetalactamase Producing Acinetobacter baumannii from Central Sterile Supply Department of a Tertiary Care Hospital

Abstract

Acinetobacter species are increasingly isolated from hospital environment. Multidrug resistance among Acinetobacter species (resistance to more than or equal to 3 drug classes) is common. Their ability to survive in diverse environmental sources and to colonize animate and inanimate reservoirs in hospital makes it difficult to control the spread of the organism. Acinetobacter is increasingly isolated from Intensive Care Units (ICU) and Central Sterile Supply Department (CSSD) of the hospital. Acinetobacter shows carbapenem resistance, mediated by serine carbapenemase and metallobetalactemase production.

Environmental bacterial surveillance of Central Sterile Supply Department (CSSD) at a tertiary care hospital in Bangalore.

A survey was conducted in central sterile supply department(CSSD) of a tertiary care hospital involving environmental sampling twice a month for a period of six months from January 2015 to June 2015. Swabs were taken from different areas of CSSD like wall, floor, packing table, racks, sink, sterile containers(outer surface) etc. Isolation and identification of bacteria was done according to the standard procedures. Antibiotic sensitivity was done by Kirby-Bauer's disc diffusion method, carbapenamase production was detected by modified Hodge test and double disc synergy test (imipenem and imipenem EDTA) according to CLSI guidelines.

Out of 12 surveys, two times Acinetobacter baumannii was isolated from wall and packing table of CSSD 2 times, amounting to total of 4 isolates. All the 4 isolates were found to be resistant to all classes of antibiotics including the drugs imipenem, meropenam and colistin. All the 4 isolates were positive for double disc synergy test and negative for modified Hodge test (MHT) indicating metallobetalactamase production.

Acinetobacter baumannii is increasingly isolated from sterile areas of hospital like CSSD and is showing metallobetalactamase production which calls for strict infection control practices.
