Psychological and neuropsychological evaluations are rendered invalid by ‘less than maximal performance on testing’. However a structured approach to the evaluation of effort and/or biased responding is not common, particularly in clinical settings. The case described highlights the difficulties faced in the detection of biased responding in a rehabilitation setting. The discussion summarises the current international recommendations, and outlines a structured flowchart for the evaluation of sub-optimal performance, biased responding, and malingering.
