Her 2/neu over expression has been studied in various range of tumors such as breast, bladder, lung, colon etc. Use of Trastuzumab, monoclonal antibody a tyrosine kinase inhibitor against Her 2 is well known in targeted treatment of breast carcinoma. Results of TOGA trial proved its usefulness in gastric and gastro oesophageal carcinomas as well. This discovery lead to many studies conducted worldwide studying expression of Her2 in gastric and gastro oesophageal carcinomas so as to select patients who can benefit from Trastuzumab treatment. This study was conducted in our hospital to study the expression of Her2 in gastric and gastro oesophageal carcinomas.
A total of 39 cases, of which 31 cases were gastric carcinomas and 8 cases were of gastro oesophageal carcinomas were included in study. Histopathological examination was followed by immunohistochemistry in all cases. After immunohistochemical staining, scoring was done according to guidelines given by Hoffmann et al and expression of Her2 was analysed.
Of 39 cases of gastric and gastro oesophageal carcinomas, 15cases (38%) were Her2 negative (score0& score 1+), 12 cases (31%) were equivocal (score 2+), 12 cases (31%) were Her2 positive (3+). Majority of the Her2 positive cases were from diffuse type (75%), intestinal type constituted about 25%. Percentage wise both intestinal (32%) and diffuse (30%) showed almost equal positivity.
Approximately 31% of patients with primary gastric and gastro esophageal carcinomas were positive for Her2 on IHC. Her 2 over expression was associated with GEJ site commonly, both intestinal and diffuse type showed equal positivity. These patients can be a cohort which can be benefited by Trastuzumab treatment.
