Thalamic Glioma-Astrocytic Neoplasm: An Analysis of Clinical Aspects and Histopathological Evaluation of 20 Cases with Reference to Patient Age

Abstract

Thalamic glioma is one of the challenging tumors to manage. A retrospective study of 20 cases of thalamic glioma, including adult and pediatric patients admitted at Civil Hospital, Ahmedabad over the period of 6 years (from January 2011 to December 2017) was done.

To analyse the clinical features of thalamic glioma with reference to patient age.

To study histopathological evaluation and grading of thalamic glioma.

Between the years 2011 and 2017, several patients with cerebral glioma were admitted at our institution. Gliomas which arose primarily in the thalamus were selected from these cases based on histopathological diagnosis. Tumours which primarily arose in the epithalamus, optico-hypothalamic region, midbrain, basal ganglia, adjacent white matter, or the third and lateral ventricles were excluded. As a result, a total of 20 patients (age at diagnosis: 10–70 years; median 40 years; male: 12; female: 8) were selected and formed the basis of this retrospective study.

In present study, 20 cases, which presented with glial tumors, were evaluated. 3 cases were of Pilocytic astrocytoma (grade I), 5 cases were of Diffuse astrocytoma (grade II), 6 cases were of Anaplastic astrocytoma (grade III) and 6 cases were of Glioblastoma multiforme (grade IV). There were 12 males and 8 females with the age ranging from 10 years to 70 years. Among the 20 patients, 10 patients had headaches (50%), 6 patients had vomiting (30%); 8 had motor weakness (40%).

Thalamic gliomas-Astrocytoma seem to have a slight predilection for occurrence in males than females. Majority of cases presented in the adult patient group. On the basis of Histopathological diagnosis, high-grade gliomas (Grade III and IV) seem to have more incidence rate than low-grade gliomas. The most common symptoms and signs were those related to increased intracranial pressure (headache and vomiting) and hemiparesis (motor weakness).
