Study of Oral Lichen Planus with Histopathological Examination & Clinical Correlation

Abstract

Oral lichen planus(OLP)is one of the rare chronic inflammatory, non infectious, oral mucosal disease that affect the stratified squamous epithelium in adults. It is considered as precancerous lesions by many authors.

The aim of this study was to analyze the cases of OLP in terms of age, gender, site, clinical forms & histopathological findings.

Total 25 cases were observed in the present study. Incisional or punch biopsies were taken in all patients with clinical diagnosis of OLP and biopsy tissue was fixed with 10%neutral buffered formalin & processed by routine paraffin processing. H&E stained slides were prepared. Microscopical examination was done to validate clinical diagnosis of OLP.

A total of 25 patients with histologically confirmed OLP diagnosis were analysed. There were more females(N = 17)than males(N = 8)and majority of patients were between 30–50 years age group. Buccal mucosa was the most common site(56%)followed by tongue(16%). The reticular form (56%) was the most prevalent clinical form followed by erosive-ulcerative form (36%). Liquefaction degeneration of basal cells and band-like zone of cellular infiltration that is confined to the superficial part of the connective tissue, consisting mainly of lymphocytes were observed in all patients(100%)

The present study revealed the predominance of OLP among middle-aged female population with most commonly involvement of bilateral buccal mucosa. The most common form is reticular type. The histological findings characteristic of oral lichen planus are Liquefaction degeneration of basal cells and band-like zone of cellular infiltration that is confined to the superficial part of the connective tissue, consisting mainly of lymphocytes.
