Posterior capsule opacification (PCO) or secondary cataract refers to the opacity that develops in the posterior capsule after cataract extraction. Extra capsular cataract extraction (ECCE) involves removal of all the lens fibres, leaving behind an intact posterior capsule and peripheral anterior capsule of the capsular bag for IOL implantation. However, along with the residual capsule, regenerative lens epithelial cells (LEC) are also left behind. PCO has been the most common visually disabling postsurgical complication of modern cataract surgery and has important medical, social and economic implications. PCO was indeed noted by Sir Harold Ridley in the first ever intraocular lens (IOL) implantation. 1 The present article reviews literature related to incidence, aetio-pathogenesis, treatment and prevention, highlighting current concepts related to PCO.
