A Randomized Controlled Trial of Sublingual Misoprostol 400 μg Versus Injectable Methyl Ergometrine 0.2 Mg in Active Management of Third Stage of Labour

Abstract

The study was conducted to compare the efficacy of Sublingual Misoprostol (400 μgm) with Methyl Ergometrine (0.2 mg IV) in the active management of third stage of labour.

A randomized clinical trial was performed on 200 women to receive either sublingual misoprostol (400 μgm) or Methyl Ergometrine (0.2 mg) for the active management of third stage of labour.

The mean haemoglobin level at the time of admission and at 48 hours after delivery was comparable between the two groups. The difference in fall of mean Hb level after 48 hours of delivery in Misoprostol group and Methyl ergometrine group was not statistically significant.

The mean blood loss in the third stage of labour in Misoprostol group (126ml) was comparable to Methyl Ergometrine group (124.14). The difference of mean blood loss between the two groups was not statistically significant (P > 0.05 i.e. non- significant).

The incidence of postpartum haemorrhage between misoprostol group and Methyl Ergometrine group was not statistically significant. (P > 0.05 i.e. non- significant) Vomiting, fever, shivering and metallic taste were the more significant side effects in misoprostol group.

Misoprostol is an effective safe and stable alternate to standard oxytocic in preventing the atonic PPH in active management of third stage of labour.
