Spectrum of Intestinal Obstruction in a Semiurban Area of Western UP, India

Abstract

Intestinal obstruction is a commonly encountered surgical emergency characterized by obstruction to the forward propulsion of bowel contents due to varied causes resulting in unacceptable burden on patients and family by its morbidity and high mortality also on the financial structure of family and society at large.

Study done at Muzaffarnagar Medical College, Muzaffarnagar, UP from September 2015 to December 2016, 75 patients were included in study. After routine and hematology and radiological investigations patients were stabilize and were reassured for further course of action. These patients were followed up for there presenting complaints. Radiological findings at presentation intraoperative findings and course in hospital, all the findings were chartered.

Male to female ratio is 1: 1.03 in study population of 75 with age group 18–60 yrs. Presenting complains abdominal pain tops the list of presenting complaints. 62% of population had multiple air fluid levels. Intraabdominal adhesions (45.35%) was the leading finding followed by stricture (20%) as the cause for obstruction there was statistical significance in association of previous surgery and adhesions in patients requiring surgery. Operative steps/procedure required release of adhesions and bands(38.65%) followed by Ileo-Ilieal resection anastomosis (22.6%) herniorraphy (13.33%) were the top procedure performed among female population gynecological surgery predominate as cause of adhesive obstruction.

Male to female ratio almost same.

Small bowel obstruction is common secondary to increase in abdominal/pelvic surgery abdominal Koch's.

Plan x-ray of abdomen still is the most effective diagnosis too available.

CT scan done preoperatively in a highly selective group of patients.

Early intervention also reduces morbidity and mortality post operatively.
