To evaluate number of cases of safe ear following single stage Tympanomastoid surgery for unsafe csom and importance of adjunct otoendoscopy in complete removal of disease from difficult to reach areas like Sinus tympani and Facial recess.
A prospective study of 40 patients for three years at Post-graduate Department of Otorhino laryngology and Head & Neck Surgery Govt. Medical College, Srinagar.
An ABG gap closure of up to 10dB was achieved in 3/8(37.5%) patients using autologous ossicles alone, an ABG closure of up to 10dB was achieved in 3/16(18.7%)patients using autologous ossicles with conchal cartilage and an ABG closure of 10dB achieved in ¼(25.0%)patient in whom Teflon piston with conchal cartilage was used. Among the patients with conductive hearing loss an ABG closure of 11 to 20dB was achieved in 1/8(12.5%)patient in whom autologous ossicles were used and in 2/16(12.5%)patients in whom autologous ossicle with conchal cartilage was used. An ABG closure of 21 to 30dB was achieved in 2/16(12.5%) patients in whom autologous ossicles with conchal cartilage was used and in 1/8(12.5%)patient in whom autologous ossicles alone were used.
A perfectly performed CWD mastoidectomy with tympanoplasty, with or without ossiculoplasty in same sitting give good and acceptable post operative results as far as the hearing improvement and dry and safe ear is concerned.
