To diagnose AFLP and evaluate materno-fetal outcome in four pregnancies complicated by this rare disorder
A retrospective analysis was carried on four pregnant women with AFLP diagnosed using Swansea Criteria at a tertiary level teaching hospital. Medical records were reviewed for presence of positive criteria and materno-fetal outcome
All women with AFLP were primigravida with mean age of 21.5 years. One woman (25%) had twin pregnancy and 60% babies born were male. Abdomen pain was the commonest symptom present in 75% (3/4)women. Leucocytosis, hypoglycemia, coagulopathy, elevated transaminases, bilirubin and urea were present in 100% of women. PPH and Acute Kidney injury was observed in 75% and 25% women respectively. Fever, encephalopathy, metabolic acidosis and IUFD were associated with high maternal mortality. High maternal mortality rate of 50% and perinatal mortality of 60% was observed
Swansea criteria is simple way for prompt diagnosis of this rare lethal condition. AFLP should be considered as differential diagnosis in all young Primigravida presenting with icterus at term or early postpartum. Prompt diagnosis, supportive care and termination of pregnancy results in improved materno-fetal outcome.
