We encountered a young female with multiple boils with cellulitis admitted in a surgical ward. Second day after admission, she developed clinical features suggestive of organophosphate poisoning. Repeated enquiry revealed inadvertent application of Malathion on the boils before admission. Dermal absorption of Malathion is not considered as public health hazard. But here we found a florid case of organophosphate poisoning following dermal application. Cause of delayed presentation has been explained.
