Envenomation with marine toxins due to stings are not so rare entities especially in the coastal regions. They are not very often brought to the notice of the tertiary care physicians since they produce minor symptoms which are treated at the primary care level. Toxin containing fish ingestion are the common sources of symptoms which rarely, endanger the life of the patient as well as produce severe morbidity that are mostly underreported. Sting related symptoms are mostly restricted to local as well as the peripheral nervous system and systemic effects. Focal central nervous system symptoms like hemiplegia as in this patient are extremely rare. Hence this case is reported.
