Non alcoholic fatty liver disease (NAFLD) comprises a wide spectrum of liver damage, ranging from simple macrovesicular steatosis to steatohepatitis, advanced fibrosis, and cirrhosis. Recent studies have focused on the presence of insulin resistance at the centre of pathophysiologic mechanisms of NAFLD. The thiazolidinedione medications like pioglitazone improve insulin sensitivity. Ursodeoxycholic acid has cytoprotective and immunomodulatory effects. Till date no effective medical therapy is available which can cure all the patients with non-alcoholic fatty liver disease. Pharmacologic therapy aimed at the underlying liver disease holds promise. With several drugs to choose between, it is essential to evaluate the effectiveness of these drugs against each other in the best possible way.
