Cryptococcosis is an acute, subacute or chronic fungal disease caused by encapsulated basidiomycetous yeast belonging to genus Cryptococcus which affects both immunodeficient and immunocompetent individuals. Clinical manifestations predominantly involve the Central Nervous System (CNS) followed by lungs in some patients. Disseminated disease associated with high fatality often involves the lungs, bone marrow (5%), and skin (10%) while pleural effusion and ascites are rare presentations. We report our experience with an unusual case of silently disseminated Cryptococcal infection in a newly diagnosed AIDS patient with minimal characteristic symptoms of infection leading to missed diagnosis at peripheral centre which later presented as fatal dissemination.
