Intracystic papillary carcinoma of the breast is a rare malignant epithelial neoplasm with papillary pattern surrounded by a fibrous capsule and low to intermediate nuclear grade. Clinically most patients are elderly and presents with clear or blood stained nipple discharge or a definite breast lump. Management includes complete surgical excision of the lesion with wide surrounding breast tissue. A case of a middle-aged woman presenting with a painless lump in the left breast which on histopathology was diagnosed as intracystic papillary carcinoma is reported here.
