Acrofacial Hyperpigmentation as A Sole Manifestation in A Case of Pancytopenia Due to Vitamin B12 Deficiency-A Diagnostic Alert

Abstract

Vitamin B12 deficiency presents with a spectrum of manifestations ranging from anemia to gastrointestinal disturbances and potentially lethal cardiovascular and neuropsychiatric illnesses. Diffuse acral hyperpigmentation with knuckle pad accentuation is a reliable marker of Vitamin B12 deficiency. We, hereby report a case of 49 years old male who presented with hyperpigmentation on face, hands and feet with knuckle pad accentuation. On investigations, he was found to have pancytopenia & Vitamin B12 deficiency. He showed marked improvement on administration of Vitamin B12.
