Renal metastasis is a common autopsy finding. However they are rarely identified antemortem, especially at presentation. We report a case of a 48-year-old man who presented with jaundice and masses in the liver and kidney. On evaluation he was found to have a poorly differentiated adeno-carcinoma with grossly elevated alpha-fetoprotein levels. He was started on weekly 5 flourouracil to which he responded. Patient is symptomatically better at 4 months follow up.
