Germ cell tumour is an uncommon malignancy in children constituting 1 to 2% of total childhood cancers. Testicular yolk sac tumour with retroperitoneal lymph nodes in a three year old child with Down's syndrome is reported. Such an association with trisomy 21 has been reported infrequently. Diagnosis was confirmed by histopathology and high serum alpha-fetoprotein levels (AFP). Trisomy 21 was confirmed by conventional karyotyping. Patient was treated with high inguinal orchidectomy and six cycles of chemotherapy consisting of carboplatin, etoposide (VP-16) and bleomycin. Regression of tumour was confirmed by normal serum AFP levels and normal ultrasound abdomen.
