The odontogenic keratocyst (OKC) has been the subject of great interest since Philipsen introduced the term in 1956. There has been a huge amount of progress in the elucidation of many biological aspects of OKC. However despite the great number of reports, there is considerable controversy with regards to its true nature. The OKC shows clinicopathological features of both a cyst and a benign neoplasm and is now known as the keratocystic odontogenic tumour (KCOT). The purpose of this paper a systematic review of the literature pertaining to the clinical features, histopathology, radiological features, recurrence, prognosis and widely accepted methods used for the treatment of OKC and current concepts in its true nature/behavior.
