Perforated peptic ulcer perforation involves sudden onset of pain and exhibits sign of perforation. A posterior perforation is rare presentations and awareness of such entity is important in surgical practice. In the present case report, perorated duodenal peptic ulceration with its intestinal complication in gangrene of the Right hemi colon has been highlighted.
