The objective of this study was to evaluate the diagnostic reliability and efficacy of invivo staining with toluidine blue, in the detection of oral epithelial dysplasia in patients with oral leukoplakia.
The study comprised of 50 clinically suspicious cases of oral leukoplakia. All lesions were submied to staining with an aqueous solution of 1% toluidine blue followed by biopsy and histologic analysis.
The sensitivity and specificity for the detection of dysplastic lesions by this test were 69.77% and 85.71%, respectively. The positive and negative predictive values calculated were 96.77% and 31.58%.
Toluidine Blue staining was found to be highly reliable in the detection of severe and moderate dysplasias but its use is questionable in the detection of mild dysplasias, owing to high false negative results. It is a simple, inexpensive and sensitive chair side test; a valuable adjunct to diagnostic process but not a substitute for either clinical judgment or biopsy. It has high feasibility, especially in low socioeconomic areas.
