Effect of Intrathecal Dexmedetomidine on the Characteristics of Bupivacaine Spinal Block in Orthopaedic Surgeries

Abstract

The aim of this study was to evaluate the onset and duration of sensory and motor block, hemodynamic effect, postoperative analgesia, and adverse effects of dexmedetomidine given intrathecally with hyperbaric 0.5% bupivacaine.

Sixty patients undergoing elective lower limb orthopaedic surgery under spinal anesthesia were selected. Inclusion criteria were American Society of Anaesthesiologists (ASA) physical status I or II, either sex, age 20–60 years, presenting for lower limb orthopaedic surgery. The study population was randomly divided into 2 groups with 30 patients in each group. Patients allotted to Group D received 0.5% hyperbaric bupivacaine 15mg + dexmedetomidine 12.5μg and Group B received 0.5% hyperbaric bupivacaine 15mg + normal saline.

Onset to sensory block was significantly shorter in group D (2.60±0.56min) than in group B(4.17±0.65min). Block regression was significantly slower with addition of intrathecal dexmedetomidine(group D) as compared to group B. The time to reach Bromage 3 motor block was significantly shorter in group D (5.0±0.9min) than in group B(7.9±0.7min).

Intrathecal dexmedetomidine significantly prolonged sensory and motor block with stable hemodynamic condition and good patient satisfaction.
