Diagnosis and prognosis by functional strength and ability in patients with low back pain

Abstract

Low back pain (LBP) is a multifactorial disorder, its treatment varies considerably. Many qualitative diagnostic tools are available but their efficacy till date is not clear. The objective of this study is to give a quantitative diagnostic of non specific LBP from functional strength and ability.

82 subjects (31 Normal and 51 LBP patients) were selected for this study. Baseline physical characteristics (Age, Weight, Height, WC, HC and BMI) of normal healthy subjects and patients were taken. Functional strength Back Pressure Changes and Abdominal Pressure Changes (BPC and APC) and ability (Walking, Climbing and Stand ups) of normal healthy subjects and patients were assessed on 0 day and 90 days. Diagnostics of each functional variables were assessed by ROC curve analysis and discriminating ability by binary logistic regression analysis.

The results showed that functional strength (back pressure changes: BPC; and abdominal pressure changes: APC) have diagnostics ability. The BPC of functional strength show 100% discriminating ability and at criterion value (threshold) of d” 25 it is discriminating cases with 100% sensitivity and specificity. Functional ability (Walking, Climbing and Stand ups) except Stand ups show good prognostic as these significantly correlated (either P<0.05 or P<0.01) with the Pain severity a reliable indicator of LBP.

The findings of this study may be helpful in the management of LBP and recommend clinicians to adopt both pressure changes (BPC and APC) for diagnostic and ability for prognostic. Through this study we will enable to judge and identify the genuiness of the leave taken by employ for the lesion of backache.
