The goal of endodontic treatment is to debride the canals and seal them with an inert filling material. The clinician should know the morphology and anatomy of individual teeth and the possible variations that can be present. Missed canals harbour bacteria and can be the cause of infection leading to post endodontic pain. This case report demonstrates an endodontic retreatment case of mandibular right first molar. During the initial treatment, three canals (mesiobuccal, mesiolingual and distal) were located. CBCT revealed an untreated distolingual root (Radix entomolaris) that was subsequently treated.
